University Award Nomination Form
Please complete the following form to nominate individuals for a University Award at Homecoming.
Note: Complete as many questions as you are able. The Alumni Relations Records team can research any missing information.
Nominations for 2022 Alumni University Awards are due by February 1, 2022.
The nomination process takes place over a few months. Around late spring, we will notify you if your nominee is selected and has accepted. We thank you in advance for your patience regarding nomination results.

Distinguished Service Award:
Awarded to BYU alumni who have given extraordinary service in the areas of their professions, community, nation, church or BYU. We are looking for balanced individuals who have offered service in several areas.
Honorary Alumni:
Awarded to individuals or couples who have not attended BYU but have rendered outstanding and significant service to BYU.
Criteria:
- Be distinguished in his or her chosen business, profession, or life's work.
- Be a person of such integrity, stature, demonstrated ability, and renown that the faculty, staff, students, and alumni of the university will take pride in, and be inspired by, his or her recognition.
- Have demonstrated a continuing interest in and loyalty to Brigham Young University.
- May not be a current BYU employee.
- May not have been an Alumni Board member during the past three years.
- May not be currently serving a mission for the Church of Jesus Christ of Latter-day Saints.
- May not be a current General Authority.
- May not be running for an elected political office.
- Deceased individuals may not be nominated.
